We are seeking an experiencedand qualified Advanced Practice Nurse with prescribing abilities and a specialinterest in diabetes. In this role, you will play a pivotal part in deliveringexceptional clinical care to our practice population, working autonomouslywithin a multidisciplinary team.
Main duties of the job
About the Role: As a Specialist Diabetic Practice Nurse, you will:
1. Provide advanced care for patients with uncontrolled and complex diabetes.
2. Identify, manage, and support patients at risk of or with poorly controlled diabetes.
3. Develop and implement care plans, prescribe within your competence, and support junior staff.
4. Deliver health education, audit care standards, and contribute to service development.
5. Undertake occasional home visits and liaise with secondary care colleagues and agencies.

JobPurpose
To provide a high quality, easily accessiblenursing service to the practice population, making best use of clinical skillswith a focus on diabetes, within the practices enabling the provision of adiverse range of services within the competence of the post holder. Working at an advance level and focusing onthe prevention, management, and treatment of Diabetes.
GeneralRequirements
To provide clinical care to the practice population registered with PrimaryCare Sheffield Practices.
To work autonomously as part of themultidisciplinary team, managing own workload, to include assessment,development, implementation, and evaluation of care in complex patients.

PrincipalDuties
Delivery of clinical care and healtheducation to the practice population.
Provision of clinical services in Diabetes,including advance care of uncontrolled, complex patients.
Prescribing within clinical competence.
Identifying, managing, and supportingpatients with poorly controlled diabetes or at risk of diabetes.
Have formal qualifications in Diabetes and aproven history of working with diabetic patients.
Have a special interest in Diabetes and to keepup to date with these skills.
Willing to undertake future training in complexlong term conditions, such as dementia.
Working at an Advanced level. Reviewing and acting on blood results forlong term conditions. Initiating carefrom these results and prescribing accordingly.
Creating patient care plans and supportingjunior staff in implementing these plans.
Providing educational and counsellingsupport to patients and junior staff members.
Using internal, local and national policies,Assess, plan and organise patient care, including evaluation of need,adjustment of care plans and recall.
Audit of care provided, aiming for a highstandard of care and achieving targets for Quality Outcomes Framework.
Using and implementing research in thepractice to ensure up to date clinical care is given and development ofservices.
Working to continually improve QOF data.
To maintain accurate and contemporaneousrecords.
Planning and organisation of clinics indiscussion with other team members.
To liaise and communicate with other teammembers, secondary care colleagues and other agencies where appropriate.
To educate other team members both clinicaland administrative, as appropriate.
To be involved in the training anddevelopment of student nurses whilst on placement.
Willing to undertakeoccasion home visits if required.
To provide nursingleadership at sites, including undertaking PRDs for staff.
