We are committed to expanding our existing 7 day acute frailty services further with the addition of a focused frailty assessment Unit in collaboration with our community colleagues a hospital at home service across County Durham and Darlington. We therefore have opportunities for colleagues to not only work in, but be involved in the development of the services.
Main duties:
1. Conduct initial triage and assess patients who present to the Frailty assessment unit.
2. Perform a comprehensive assessment of the patient's condition, including physical exams, history taking, and diagnostic tests.
3. Prioritize and provide appropriate care, ensuring patients receive timely interventions.
4. Administer medications, treatments, and IV therapy as needed.
5. Collaborate with doctors and other healthcare professionals to quickly diagnose and treat patients.
6. Perform or assist in minor procedures, such as wound care, suturing, and administering injections.
7. Ensure patients are stabilized and treated efficiently.
About us:
We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
Job responsibilities:
1. Oversee the delivery of patient care within the clinical area.
2. Ensure that care is delivered in accordance with clinical standards and patient needs.
3. Supervise and assist in the administration of medications and treatments.
4. Maintain detailed patient care records and update care plans regularly.
5. Lead, mentor, and supervise a team of nurses and healthcare assistants.
6. Ensure compliance with regulatory requirements, hospital policies, and safety protocols.
7. Act as a point of contact for patients and their families, addressing their concerns and needs.
8. Stay updated with advancements in nursing practices and healthcare technology.
Key Qualifications:
1. Registered Nurse (RN) with a valid practicing license.
2. Postgraduate Qualification in Nursing or a related clinical specialty (preferred).
3. At least 2-5 years of nursing experience, with some in a leadership or management role.
Skills and Attributes:
1. Excellent organizational and time-management skills.
2. Compassionate and empathetic, with a strong patient-centered approach.
3. Ability to manage stressful situations and make decisions under pressure.
4. High attention to detail and accuracy in patient care documentation.
