Job Overview
To work as a Band 6 Community Mental Health Practitioner in the Dementia Intensive Support Service in Mid Essex, which is part of the Older Adult Community Mental Health, Dementia and Frailty Service. The post holder will be responsible for providing rapid assessments for individuals living with dementia or suspected dementia in crisis situations requiring home treatment to avoid hospital admission.
The post holder will also support the newly forming Functional Intensive Response and Support Team (FIRST), providing assessment, support, and appropriate care planning to prevent unnecessary hospital admissions and support early discharge when individuals are admitted.
Main Duties of the Job
* Provide high quality, evidence-based, holistic care to patients in line with your professional code of conduct and Trust policies.
* Assess, plan, implement, and evaluate packages of care based on individual service user needs, under the guidance of the Advanced Nurse Practitioner.
* Supervise and manage junior team members within your professional group and team locality in line with agreed Trust HR processes.

Detailed Job Description and Main Responsibilities
* Flexible approach to work and ability to respond in crisis situations.
* Knowledge of dementia and an understanding of frailty in older adults.
* Knowledge of crisis interventions/home treatment and admission avoidance practices.
* Ability to prioritize according to presenting risks.
* Conduct assessments, risk assessments, and implement care plans.
* Knowledge of health legislation, Mental Capacity Act, Mental Health Act 1983, and safeguarding.
* Experience in the performance management and supervision of lower banded staff.
* Excellent IT skills (Word, Excel, PowerPoint, Outlook).
* Current UK driving license and access to a car for work purposes.
Person Specification
Education
* Registered Mental Health Nurse or Registered General Nurse.
* Good standard of written and spoken English.
* Good standard of education.
* Postgraduate qualification or commitment to work towards non-medical prescribing qualification.
Skills
* Literate in IT/Computer Skills.
* Good oral communication skills.
* Ability to manage and coordinate multi-disciplinary meetings.
* Undertake a range of clinical duties.
* Plan and prioritize own and team workload.
* Experience of working in the Mental Health environment.
Knowledge
* Understanding of assessments and care reviews in line with mental health care clusters.
* Knowledge of current best practices within nursing in mental health.
