Job summary
You will be workingcollaboratively with our Multidisciplinary Diabetes Footcare and CommunityPodiatry teams. The role will encompasscommunity team development, staff recruitment, offering clinical leadership whilstproviding a high level of clinical carein the specialist area of wound care and tissue viability.
We are looking for anexperienced Podiatrist with a passion for clinical excellence and with evidenceof previous positive outcomes, to work with patients under the care of theCommunity Podiatry Team. The successfulcandidate will need to be an autonomous practitioner, with experience ofworking with complex foot care needs as well as diabetes experience.
You should be an effectivecommunicator with the drive and enthusiasm to work with your team members toexpand their roles. You should be able to negotiate and collaborate effectivelywith senior clinicians and medical colleagues and facilitate the delivery ofexcellent care.
You should be able to manage acaseload, using evidence based/ patient centred principles to assess, plan anddeliver individualised treatment programmes to highly complex patients anddelegate responsibility as appropriate within the team and to wider Communityservices. You will also need to be ableto manage information to inform service development and to demonstrateachievement of target.
Main duties of the job
Within the post you will provideclinical leadership across two of the community podiatry teams within WorcesterCity and Kidderminster area with support of other TV podiatry leads withinthose teams. This is a full time post and as part of the role and participate in an on-call rota to coverurgent referrals received at the weekend into the Multidisciplinary DiabetesFootcare Team. This equates to 6-8 sessions per annum.
1. Providespecialist podiatric care to patients with active foot conditions related towounds and tissue viability in clinic and in their homes.
2. Assessand formulate treatment plans.
3. Reviewpatient care plans.
4. Managereferrals effectively through triage andprovide clinical advice to other health care professionals.
5. Workwith Band 5 and 6 to develop competencies and confidence.
6. Providesupport to students.
7. ConductABPIs
8. Workclosely with consultants from vascular, orthopaedics and diabetes, MDFT,nurses and GPs
9. Conductnail surgery as per team rota.
10. Empowerpatients and support staff.
11. Liaisewith Team leads and county wide TV leads.
12. Participatein a consultant led clinics within the acute alongside the vascular team.

Job responsibilities
13. Provideexpert podiatry input as part of the Community Podiatry Team.
14. Work closely with Consultants, The MultidisciplinaryDiabetes Footcare Team, specialist Nurses and General practitioners to managepatients with active foot conditions.
15. Provide specialist podiatric care for patients withactive foot conditions related to wounds and tissue viability.
16. Assess, and formulate specialist treatment plans basedon patients individual needs, having to complete complex clinical analysiswhich may require consideration of several components which may be conflicting.The emphasis of this process will be on the relevant area of expert specialty.
17. Directly refer for appropriate diagnosticinvestigations.
18. Liaise with patients, their carers and others with aview to education, empowerment and improvement of clinical outcomes.
19. Manage referrals into the Community Podiatry Team andensure all referrals are appropriately triaged within 2 working days andallocated appropriate appointments if necessary.
20. Assess, diagnose and formulate specialist treatmentplans based on patients individual needs with great emphasis on the at riskfoot those suffering from Diabetes Mellitus, Peripheral Vascular Disease,Rheumatoid Arthritis and Peripheral Neuropathy
21. Act as a source of expert advice, supervision andeducation for fellow podiatrists and other healthcare professionals, includingGeneral Medical Practitioners
22. Liaise with the Team Leader and others podiatry Tissueviability leads to ensure a high-qualityseamless patient delivery service in Worcestershire.
23. Deputise in the absence of the Team Leader.
24. Utilise the full scope of clinical practice includingwound management, total and partial nail ablations, carried out under localanaesthetic.
25. Be actively involved in, and with a commitment to, aclinical philosophy of care that puts an emphasis on patient empowerment andself-care, supported by health education activities.
26. Participateactively in peer review and reflective practice sessions.

27. 5 years experience showing good evidence of post qualification experience treating high risk patients within podiatry or part of a MDT
28. Experience of carrying out expert diabetic assessment and examination
29. Experience of carrying out biomechanical assessments and examinations, ability to prescribe, cast and manufacture orthotics
Desirable
30. Post graduate training to masters level in relevant speciality
Knowledge
Essential
31. Expert clinical knowledge of the management of a variety of pathologies including diabetes, peripheral vascular disease, renal and rheumatological conditions. This will include the skilled use of equipment for specialist vascular and neurological assessment and an up-to-date knowledge of wound care.
