Training and supervision will be provided to enable individuals to develop their skill set, and integrated working within the team will support case management. The post holder will: carry a caseload of patients and be involved in the delivery of consultation, advice and MDT support through holistic dementia palliative assessment and person-centred care planning. primarily cover the Chesterfield area. have a current full driving licence as role will require travel across Derbyshire. provide specialist support to both patients living and dying with dementia and their carers with identified palliative care needs. support GPs and primary care teams with complex care planning. be focussed on ensuring the person remains at home (wherever home is) and avoiding unnecessary acute care admission. support discharges from acute hospital beds and pathway 3 beds. carry a caseload of patients and be involved in the delivery of consultation, advice and MDT support. liaise direct with colleagues from the Older Adults Community Mental Health Teams and the Dementia Rapid Response Team.
