Description
This is the entry level professional position in community based psychosocial rehabilitation services for individuals with the most severe mental illness. The Case Manager plans, coordinates, provides and monitors therapeutic, rehabilitative, support and community integration services for adults with severe psychiatric disability designed to assist these individuals to integrate as independently and fully into the life of the community as possible. Case Managers work independently and must demonstrate excellent clinical judgment, decision making and safety consciousness to assure safety and appropriate treatment for consumers in the community. Works independently in community settings with consumers or leading therapeutic groups.
Requirements
1. Bachelors Degree in psychology, social work, psychosocial rehabilitation, education or related human services field or Masters Degree in Psychology, social work, psychosocial rehabilitation or related human services field preferred.
2. 2 years experience in community based psychosocial rehabilitation treatment or Case Management Services.
3. Direct lived or life experiences with close family members may substitute for some or all of the experience requirement.
