Responsibilities and Duties: Responsibilities and duties will include, but may not be limited to the following:
1. Provide assessment, diagnosis, and treatment of both acute and chronic conditions, acting as an autonomous clinician with support from the GPs. This includes initiating appropriate treatment and/or referral, including admission to hospital if required.
2. Assess both children and adults.
3. Act as practice liaison to our two residential care homes for adults with learning disabilities, arranging regular weekly visits to assess both acute and chronic conditions and undertake annual LD reviews.
4. Proactively manage housebound and frail patients with input from our practice care co-ordinator, identifying early clinical needs to reduce deterioration and admission. This includes home visits to assess conditions.
5. Work with outside agencies to ensure appropriate care.
6. Undertake a share of daily home visits if needed.
7. Maintain adherence and familiarity with locally agreed healthcare pathways and signpost appropriately to other services.
8. Support GP administrative workload with suitable training and support.
9. Support and lead the practice nursing team, including mentoring as appropriate.
10. Proficiently use the clinical IT system for safe and thorough documentation of patient care and extract information for clinical governance.
11. Collaborate with other members of the healthcare team, such as the District Nursing Team, Mental Health Services, staff in Care Homes, Ambulance Service, and Secondary Care.
12. Engage in self-appraisal and ensure practice is within good clinical governance and based on sound up-to-date clinical evidence.
13. Identify and assess any risks associated with patient care or to oneself and communicate these risks to the appropriate clinical manager.
14. Ensure effective time management and prioritization of workload.
15. Work within own scope of practice and understand when to refer to GPs.
16. Participate and contribute to clinical meetings.
17. Work within the requirements of Data Protection legislation as it applies to patient records.
18. Maintain current professional registration and indemnity.
