Thisis an exciting opportunity to join an existing multidisciplinary MedicinesOptimisation Team, comprising pharmacists, pharmacy technicians and adietitian, working within Kirklees Health and Care Partnership, supportingpractices in Kirklees. The locality covers Greater Huddersfield and NorthKirklees, with 64 practices situated across urban and semi-rural areas.
Youwill collaborate with colleagues to support the development and implementationof the Medicines Optimisation initiatives at practice and PCN level, which willbe delivered via a hybrid work model, combining time spent in practices, athead office / other ICB premises and working from home.
Youwill have excellent interpersonal skills and the ability to operate as aneffective team player, building positive and productive relationships withsystem partners such as GPs, other healthcare professionals and practice staff.

Main duties of the job
Asan advanced pharmacist, you will work with allocated practices, implementinginitiatives to improve the cost-effective, evidence-based use of medicinesacross the locality. You will contribute to the development of the MedicinesOptimisation strategy, policy and guidelines, and will be responsible forleading on agreed clinical areas within the Medicines Optimisation Team. Youwill also have the opportunity to work with colleagues from MedicinesOptimisation teams from other Places within the ICB.
Youwill need to be able to build and sustain relationships with colleagues in ICBteams other than the Medicines Optimisation team, such as the Quality andPrimary Care teams, and other colleagues and professionals in primary andsecondary care, the Local Authority, Non-Medical Prescribers, CommunityMatrons, District Nurses, Community Pharmacists, Dieticians etc.
Youwill liaise closely with the Principal Pharmacist, the Lead Technician andother Advanced Pharmacists to develop and co-ordinate work plans. You willsupport prescribers and general practice staff in improving the health of theirpatients through the rational and safe use of medicines, and the developmentand implementation of safe medicines optimisation systems. You will utiliseePACT2, OpenPrescribing and GP computer systems to identify and monitorprescribing information and produce appropriate reports for presentation toprimary care teams.

Your responsibilities will include:
To plan and co-ordinate the development and implementation of ICB Medicines Optimisation initiatives, policy and strategy documents.
To promote clinically effective and cost-effective prescribing at a practice level, to maximise health benefits from limited resources.
To lead projects on medicines optimisation that deliver Cost Improvement Plans (CIPs), cost effective use of medicines and related products/services and other projects for Kirklees Health and Care Partnership and WYICB
To influence and negotiate change with hospital pharmacists, clinicians and community pharmacy to promote and implement rational prescribing across primary and secondary care.
To present complex and contentious information to internal and external groups and committees as part of developing, agreeing, and implementing Medicines Optimisation initiatives, policies and strategies.
To influence and negotiate change with GPs and practice staff in relation to complex medicines optimisation issues at scheduled meetings and on an ad hoc basis.
To provide highly specialised advice on medicines optimisation to support pathway and service redesign as part of ICB service development and transformation work.
To actively participate in prescribing groups across the health economy, as agreed with the Head of Medicines Optimisation and Principal Pharmacist and represent Kirklees Health and Care Partnership for medicines optimisation issues
To provide highly specialist advice on medicines optimisation in response to the queries from healthcare professionals, practice staff, patients, carers and the public
