Job summary To promote and develop the Neurodevelopmental Service in CAMHS by providing clinical and professional leadership in development and implementation of evidence-based practices to support people with a learning disability. To co-ordinate and facilitate high-quality person-centred specialist health care for children and young people with learning disabilities within CAMHS and the community setting. To ensure seamless care provision between mainstream Hospital services, Primary Care services, GP surgeries and specialist learning disability services to reduce health inequalities. Providing nursing advice, education and direct support on complex health care needs of people with a learning disability to individuals, families, staff and provider services. Main duties of the job Responsible for liaising with Hospital,, Learning Disability Services, Schools, Childrens Service, CAMHS mental health practitioners and all other relevant carers in order to facilitate an effective health care pathway for people with a learning disability. Enhance care delivery by promoting a whole system approach to achieve positive outcomes within a multi-agency/disciplinary partnership. Co-ordinate and participate in multidisciplinary/agency team meetings, hospital admissions, case conferences, pre discharge meetings and GP practice meetings. Engage with all health care services to promote the rights of individuals with a learning disability to access an equitable service and reasonable adjustments are made in order to meet individual needs. Participate in the development and review of policies and procedures to ensure the health needs of people with a learning disability are considered and in particular, to develop processes and pathways within referrals and admissions to health care services Liaise with Hospital, Acute Services and GP practices to ensure people with a learning disability are appropriately identified and recorded, supporting staff to develop competencies to carry out routine health assessment, screening and consultation for children and young people with a learning disability where required.
