Hours
Full Time. 150 hours worked over 4 weeks, in 12 hour shifts (8am -8pm). Including weekends and public holidays.

It is an exciting time at Trinity as we launch our new community services, which make up our new clinical hub. The new services have been developed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population and maximise the flexibility and responsiveness we can offer.
We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of a Care Coordinator within the Clinical Hub Services. This post is suited to a Care Coordinator with some experience or knowledge of Coordination of patient care in various settings. Supporting patients to achieve their preferred place of care, avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital.
You will be a key member of the Clinical Hub Services, and the wider multidisciplinary team working across Royal Trinity Hospice’s community catchment area supporting patients with urgent and complex specialist palliative care needs wherever they call home.
Key Responsibilities of a Royal Trinity Hospice Care Coordinator
* To support the functions of the Clinical Hub including:
* First Contact
* Rapid Response
* Specialist Community Nursing
* To support the single point of access to all Trinity’s care services
* To be the point of contact for all queries from patients; their carers and health and allied health care professionals
* To ensure patients, their friends and families are allocated to the right service, with the right professional, at the right time.
Skills and Experience Required
* Competent IT Skills
* Effective communication: interpersonal and organisational skills, face to face, in writing and over telephone
* Strong organisation skills and attention to detail
* Previous experience of working in a healthcare environment and/or end of life care patients
* Willingness to learn new skills.
* Demonstrates the desire to work in a team and with patients who have specialist palliative care needs or who are approaching end of life.
