To work in collaboration with GP practices, community, statutory, and voluntary sector services to deliver a coordinated high-quality social prescribing service. To assist individuals to identify their needs and work towards solutions that are right for them. The Social Prescribing Practitioner will co-productively draw up a personalised plan supporting patients to access and engage with the extensive range of support available in the community. Social Prescribing is a short-term intervention service working towards building a diverse and sustainable network of support for those using the service. The social prescribing approach empowers individuals to take control of their own health and wellbeing. Your role would involve working on a one-to-one basis, offering person centred appointments/care to support patients needs, connecting patients to community groups and statutory services for practical and emotional support when appropriate. Your role is central in developing a relationship with patients, you will resourceful and creative to find the most appropriate support and build patient relationships. You should have experience of working positively with people facing social and emotional challenges. You will be working with people from diverse cultural and social backgrounds and will need to work confidently and effectively in a diverse and challenging environment. You will be required to undertake home visits so you will be required to hold a full clean driving licence and access to your own vehicle with insurance coverage for business use. Were looking for someone who has: Experience of working with people on a one-to one basis in an information, advice and guidance health care sector The ability to plan, develop and manage your own caseload to ensure patient objectives are met To provide person-centred support, agreeing well-being action plans with individual participants, identifying and planning support needs to ensure maximum engagement and improve health, wellbeing and confidence Knowledge confidentiality and information sharing protocols Knowledge and practical experience of local safeguarding processes and an ability to operate within relevant policies and procedures to safeguard our Patients Youll need to be: Well organised and resourceful with ability to work on own initiative, meet deadlines and monitor own performance Flexible within the practice to attend any meetings in relation to your role, MDTs in relation to patients on your caseload. You will be required to work over four sites within the Immingham, Keelby and Grimsby area (Meridian Primary Care Network) You will have the ability to work as part of a team, some lone working may be required (subject to risk assessment). You will be confident, outgoing and have excellent communication skills. Able to record accurate information on IT systems. You will also Be able to think outside of the box, be adaptable in ways of working, continuously improving your skills to support our patients effectively. Essential Health and social care, coaching, counselling, or other appropriate background relevant to the role Proven experience of planning and goal setting to help improve health and well-being outcomes Proven track record in relationship building skills across a variety of different networks Knowledge of Primary Care and the voluntary and community sector Experience of delivering general and/or specialist advice and information to clients An understanding of Social Prescribing and how it may contribute to health and wellbeing Good IT Skills and the ability to complete all necessary client records A confident and professional approach to working with a variety of stakeholders A proven understanding of safeguarding and ability to implement relevant policies and procedures The ability to work autonomously and to plan, prioritise work under pressure and adapt to new models of working A commitment to equal opportunities A commitment to Continuous Professional Development and undertaking relevant training opportunities when they become available.
