Responsibilities
* Provide direct services to children assessed as needing therapeutic services.
* Attend FTC and other related case conferences for children assessed as needing clinical support.
* Assist parents and children in carrying out our treatment plan.
* Make home visits and collateral contacts to meet ACS and OCFS requirements.
* Provide in-home and in-community behavior management, therapeutic, and developmental support to children with complex needs.
* Support specific behavioral goals as developed by the treatment team.
* Create, develop, and implement daily experiences for the children that promote self-esteem and socialization skills, preparing children for family unification.
* Perform child/youth care functions relevant to daily home care, ADL skills, scheduling, and setting plans for respite and crisis intervention.
* Document progress notes in CONNECTIONS.
* Other related duties as assigned by supervisor.
Qualifications
* BA/BS Degree with at least 2 years’ experience working with children and families.
* Excellent verbal and written communication skills.
* Computer proficient; knowledge of Connections is a plus.
* Bilingual (English/Spanish) is a plus.
* Hybrid: 2 days home, 3 days office.

Additional Requirements
* Authorized to work in the U.S.
* Ability to work in-person in Brooklyn, NY for at least 3 days or more per week.
* Ability to travel to other Rising Ground sites (if required).

Compensation & Benefits
The annual salary for this role is $47,840. Rising Ground offers competitive compensation based on the midpoint of the market among not-for-profit organizations of similar size.
