As a substance misuse nurse, you will manage a clinical caseload of men who misuse substances, ensuring appropriate assessment and treatment interventions are provided, and SMART goals are agreed.
You will:
1. Plan, deliver and evaluate interventions and/or treatments when there are complex issues and/or serious illness. You will discuss the treatment plan and the related needs with the service user and the wider team and will understand his/her own role in delivering interventions and/or treatments within the plan.
2. Identify any specific precautions or contra-indications to the proposed interventions/treatments and take the appropriate action.
3. Support and monitor patients throughout, promptly alerting the line manager when there are unexpected changes in individuals' health and wellbeing or level of risk.
4. Provide information to the SMS team on how individuals' needs are changing and feedback on the appropriateness of the individual's treatment plan when there are issues.
5. Respond to, record and report any adverse event or incident relating to the intervention/treatment with an appropriate degree of urgency.
6. Prepare for, undertake and record interventions/treatments correctly, and in line with legislation, policies and procedures and/or established protocols.
Main duties of the job
Good communication skills are vital as the role will include the direct management of service users on their treatment pathway. You will need to provide a holistic service and ensure that management plans are specific to individuals' needs and delivered in a way which the patient understands.
You will be expected to attend and contribute to multi-disciplinary meetings and liaise with community teams, so establishing good working relationships with both the prison and community services will be vital.
Our aim is to identify staff with a strong belief in recovery. It is important that you have a good understanding of caring for patients from all backgrounds, including an awareness of mental illness and substance misuse to identify risky behaviours.
You will be joining a team committed to improving the health and wellbeing of patients under our care, so you will need to be approachable, energetic, resilient, and adaptable to an ever-changing environment.
In addition, we are keen to develop employees and offer excellent progression through various career pathways.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
