* WORK LOCATION: Alban, Noëlville, St-Charles and Warren
* Provide a range of primary care services to the patients of the Centre de santé Univi Health Centre (CSUHC); perform client assessment, follow-ups, triage, screening, and counselling.
* Collaborate with nurse practitioners, physicians and other healthcare professionals to develop and implement individualized care plans.
* Participate in the development, delivery and evaluation of educational and health promotion programs.
* Maintain accurate and timely records and encounter data in patient electronic medical record (EMR).
* Participate in chart and case review.
* Communicate information clearly and concisely to patients and health care providers on a regular basis.
* Educate patients and their families about healthcare, treatments, and procedures.
* Participate in the assessment and reassessment of patient care needs.
* Adhere to policies and procedures as well as professional standards.
* Other duties as assigned by the Program Director.
MINIMUM REQUIREMENTS:
* Bachelor’s Degree in Nursing, or Diploma with relevant experience.
* Membership in good standing with the College of Nurses of Ontario.
* Work experience in a primary care setting preferred.
* Knowledge and ability to assess the needs of various age groups.
* Experience in the planning, implementation and evaluation of various health-related topics in a community setting.
* Superior problem-solving skills with the ability to prioritize and time manage in a demanding office environment.
* Knowledge of community resources, especially in regards to health and social services.
* Ability to work within an interdisciplinary team.
* Current certification in Basic Cardiac Life Support (BCLS).
* Bilingualism preferred (English and French).
* Solid working knowledge and ability to navigate a PC windows environment, including shared drives and Microsoft applications.
* Valid Ontario’s driver’s license and access to a vehicle, would be an asset.
