We are looking for a proactive, enthusiastic, self-motivated Assistant Practitioner to join our Continence Team.
They will work flexibly across Sunderland and South Tyneside whilst being based at based at Clarendon, Hebburn.
The role of the Assistant Practitioner is essential to provide enhanced support to the registered nurse whilst working both supervised and unsupervised within community settings with an aim to bridge the gap between the HCA and RN role.
Main duties of the job
The post holder will be an excellent communicator and work well both individually and within a team.
You will carry out planned and unplanned health (nursing) interventions within the continence service as designated by the lead clinician.
You will work both supervised and unsupervised in patients own homes, residential, nursing and other intermediate care facilities, reporting back patient progress and informing the lead clinician of any problems The Assistant Practitioner will provide advanced support to the registered nurse and will work within the defined Trust policies, procedures and competencies and treatment plans for patients creating safe, caring environment that provides privacy, dignity and respect for the patient.
