An opportunity has arisen for enthusiastic, motivated, hardworking individuals to join the Mental Health Assertive Outreach (AO) Service. A RMN or Registered Mental Health Nurse qualification is essential.
The AO Service is a community based tertiary psychosis service who work with people with severe and enduring mental health problems. We take referrals from CMHTs, Forensic Service, PIER and in-patient areas. We operate a team approach providing comprehensive care packages for people who reside in Leicester, Leicestershire and Rutland.

The working pattern for the post will be either Monday to Friday 9.00am to 5.00pm or Monday to Sunday 9.00am to 5.00pm.

Main duties of the job
The Mental Health Practitioner in the Assertive Outreach team on a day to day basis will assess, plan, deliver and evaluate care for service users who are referred to the Assertive Outreach Service, including the assessment and management of risk, working within a psychosocial / family centred framework when assessing and planning individual packages of care. You will be responsible for organising and planning your work as an autonomous practitioner managing your time effectively and liaising closely with other agencies and disciplines.
Candidates will need to demonstrate a commitment to providing comprehensive care packages for people with enduring mental health problems within their own homes.
Excellent assessment, risk management, organisational and communication skills are essential along with experience and a good working knowledge of the Care Co-ordination policy, Community mental health legislation (especially CTO's) and Hospital admission / discharge process.
The successful candidate will undertake the role of Care coordinator/Lead professional for a case load of service users with complex mental health problems who are difficult to engage and require intensive packages of care, developing therapeutic relationships with service users in order to provide support and assistance and may be subject to a Community Treatment order.
