To work in the Urgent Community response and intermediate care service, supporting Scarborough and Ryedale localities. This role will support patients to remain at home, reduce hospital admission and support discharge. Enabling patients to be as independent as possible by providing specialist physiotherapy assessment & interventions to patients with a wide variety of clinical conditions in their own homes & care homes within the local community.
