Job Description:
An exciting job role has recently become available at the Herefordshire and Worcester NHS Trust. They are looking for a new member for their Hospital Discharge Team.
The role’s purpose is to work within their professional scope of practice to actively promote values-based practice when working with others to ensure rights-based and recovery-focused approaches whilst advocating for the service user.
Responsibilities:
* Complete comprehensive assessments and personalised care which is dynamic and flexible to support service users and their families/carers.
* Provide evidence-based interventions and specialist advice.
* Work as a role model within a multidisciplinary team enhancing the unique value of a mental health practitioner.
* Deliver evidence-based interventions following NICE guidelines to promote recovery.
* Regularly review care and outcomes, supporting service users to complete clinical outcomes.
* Measure and monitor progress and adapt treatment options as required.
