An exciting opportunity has arisen for a Community Mental Health Nurse position within the Home Treatment Team - West, based in Basildon. We are seeking a motivated and enthusiastic individual who is passionate about community mental health. The post holder will join an evolving team and will be jointly responsible for the delivery of a high standard of community care, forming part of a coordinated plan of home treatment to Service Users predominantly within their own homes.
Main duties of the job
We provide intensive home support to service users aged 18 and over through a range of referral routes, which include work with our community teams, liaison services, and partner organisations to provide a range of support options for people experiencing mental health crisis. The service 'gate keeps' all admissions to acute mental health wards; we endeavour to provide care in the least restrictive manner by offering an alternative to hospital admission through intensive home treatment. Where required, the team supports planned admission to wards and facilitates timely discharge from inpatient care, working closely with wards to identify patients who could be better supported in their communities.
We are seeking a Community Mental Health Nurse to join our team of Consultant Psychiatrists, Junior Doctors, Nurses, and Support Workers providing rapid response for adults who are experiencing an acute psychiatric crisis. Job responsibilities
The post holder carries responsibility for the assessment of care needs and their development, implementation, and evaluation of programs of care for clients with mental health problems who are referred to the Home Treatment Team Service. The post holder is responsible on a day-to-day basis to the Clinical Lead but ultimately accountable to the Matron for the overall service. In addition, they will be a member of a multi-disciplinary team and work in close co-operation with wards and community teams. They will be responsible for the maintenance of standards of care within the Home Treatment service, sharing responsibility for an integrated service provision to the clients and ensuring that a good inter-disciplinary working environment is maintained.
Person Specification
Education/Qualifications
* Professional Qualification (OT, Social Work, RMN)
* Evidence of personal and professional development in contemporary mental health practice
* Educated to Degree Level in Health
* Additional Professional Qualification/ Experience - Dementia related
Knowledge
* Able to take on broader responsibilities to improve own and others knowledge and skills
* Evidence of the knowledge and skills in the use of information technology to support care and professional development
* Good understanding of the Mental Capacity Act and its application in practice
* Good understanding of person-centred care and the Recovery Model
* Good understanding of crisis resolution home treatment service provision
* Good understanding of positive risk-taking
* Good understanding of national policy and the implications for mental health e.g. crisis care concordat, parity of esteem
* Evidence of an understanding of the principle of governance and implications for professionals practice
* Evidence of knowledge of professional nursing standards
* IT competency
* Car Driver /able to travel to all required locations and achieve service activity targets
