Job summary The Community Mental Health Rehabilitation & Enablement service is an exciting service that focuses on the rehabilitation needs of service users who have complex mental health issues and require intensive support to promote social inclusion and facilitate greater independence. This service aims to enable Service Users to develop the right skills to improve their quality of life through independent living and effective management of their mental health difficulties. An opportunity has arisen to appoint a Band 6 Community Mental Health Nurse within our Bexley CMHRES team. This is a unique and exciting opportunity for an enthusiastic and innovative professional from a community mental health nursing background who has experience of working with individuals with mental health problems, in community settings and fully understands the rehabilitation, enablement and recovery models of evidence based clinical practice. Main duties of the job As a Care Coordinator working in Bexley Community Mental Health and Enablement Service (CMHRES), you will promote and deliver evidence-based care for service users with mental health conditions in their own homes and within a variety of accommodation settings. You will hold responsibility for a defined caseload of service users who have complex mental health and rehabilitation needs. The main aim of the Team is to focus on rehabilitation, engagement, relapse prevention and enablement to optimise the service users' ability to live independently and enjoy a good quality of life.
