Salary: £51,801 - £59,724 per annum (+ £3,000 per annum Annually reviewed Retention Payment)
Grade: G
Permanent Contract
37 Hours Per Week - (Office based), including possible weekend working (Saturdays)
We have an exciting opportunity for a Social Care Team Manager to join Adult Social Care Services on a full-time basis.
The successful applicant will be joining the Hospital Hub Social Work Team to facilitate safe and timely discharges from acute and community hospitals, working in a way that is responsive and proportionate to resident needs, working with people to achieve their outcomes using their strengths and their community creatively, and linking people with their communities focusing on what matters rather than what's the matter.
Working as part of the team you will be working alongside health leaders and professionals to ensure the patient is safely discharged. Weekend working may also be expected.
We are looking for a Team Manager with at least two years managerial experience to join this supportive and highly skilled team.
* You will need to be able to supervise social workers and support planners. Also, work closely with colleagues in other departments and agencies to meet the care and support needs and outcomes for our service users.
* You will become part of a team where we provide the opportunity for you to grow and develop and join a very innovative and supportive service.
* You will need to be an excellent communicator and able to work alongside colleagues in Health and Voluntary organisations.
You will join the team to facilitate integrated service delivery and broaden the scope of support we provide to our residents, while ensuring the overall service objectives for Adults, Housing & Health is achieved.
Key experience and skills:
* 5 years qualified.
* Working within Health and social care settings.
You will need to hold a recognised qualification in Social Work and be registered with Social Work England.
An Enhanced Disclosure and Barring Service (DBS) check with Adults Barred List will be required.
