Responsibilities:
* Conduct assessments under the Care Act 2014 to identify and meet eligible care and support needs.
* Develop person-centred care plans that promote independence, choice, and well-being.
* Advocate for individuals’ best interests when they lack capacity, ensuring their rights are upheld.
* Safeguard adults at risk, responding to concerns and implementing protective measures.
* Collaborate with multi-disciplinary teams to coordinate effective care and support services.
* Provide information and advice to empower individuals to make informed choices about their care.
* Maintain accurate and timely case records in line with legal and organisational requirements.
To be considered for this position, it is essential to hold a social work qualification and maintain relevant registration.
