It is essential for the successful candidate to have access to a car, have a full drivers licence, and be happy to transport customers in their car. What is great about the role? You will play a key part in delivery of the preventative hubs in Llanelli, Amman and Gwendreath, these hubs provide preventative and solution-based community support to support customers to improve and/ or maintain their health, well-being and independence through social integration and strong community relationships. As part of the lead team, you will deliver the single point of contact from which customers can access information, opportunities, and support is visible, accessible and available. You will work closely with your team and partner organisations to deliver this service. You will, receive referrals, provide information, have wellbeing conversations with customers, deliver community engagement campaigns and develop community support networks. What skills and experience do you need? We are looking for someone that is able to build great rapport and has experience of working with vulnerable people or in community development. You will have the ability to plan, deliver, review and monitor a holistic package of support and have the ability to assess the needs of our customers and assess risk and propose risk management strategies.
