About the Role
In Bracknell’s Community Mental Health Team for Older Adults, we support our most vulnerable residents using a strength-based approach; helping the person to remain as independent as possible is at the heart of everything we do. Our Social Workers have the time to get to know everyone they work with, working with people to change lives for the better.
The Community Mental Health Team for Older Adults is comprised of highly professional and supportive multidisciplinary teams. Our social care teams work together as one service and go the extra mile to ensure our residents have the right support.
We work closely with health and other partners to ensure that people remain living in their own homes for as long as possible. The teams have weekly multidisciplinary meetings, providing our residents with easy access to health and social care services that talk to each other, to provide holistic support when needed.
Support for the successful candidate will be given by the team, including regular supervision and access to training. Bracknell has a strong focus on supporting staff to learn, grow, and develop to achieve their potential.
Main Duties
You will be supported to carry out the following duties, responsibilities, and training:
* Manage a small complex caseload, organizing and prioritizing the workload effectively, with regular supervision and guidance.
* Undertake the role of Designated Safeguarding Officer on a rota basis.
* Work as part of a multi-disciplinary team, connecting people and their families to their community and social networks.
* Take a strengths-based approach to support people living with dementia to lead fulfilling lives.
* Support in delivering and maintaining high standards of performance across the service.
* Take an evidence-based approach to practice.
* Provide leadership, supervision, and peer support, acting as a role model for the experienced social work team.
* Participate actively in your own supervision and appraisal.
* Take responsibility for your own learning, including mandatory and recommended training.
* Lead on specific areas of work as required.
Qualifications
* The applicant must be a qualified Social Worker registered with Social Work England.
* At least 2 years of post-qualifying experience in statutory social services, usually in a local authority setting.
* Demonstrate excellent knowledge of best practice and legislation. Must have a professional and “can-do” attitude.
* Sponsorship is available for UK-based applicants.
