We are looking for a newly qualified or experienced Practice Nurse to join our award-winning practice.
Smear training completion & experience is mandatory to apply for this role.
The candidate will undertake all General Practice chronic disease management as well as general practice nursing duties, including child/adult vaccinations, basic wound care, and cytology (These 3 accreditations and experience are Mandatory). The candidate should be competent in all QuoF chronic disease reviews and immunisation.

Ideally, we are looking for a candidate to work approximately 25-30 hours over 5 days Fairly FLEXIBLE - ideally PM sessions across main and branch sites. Ample overtime to cover colleagues' holidays available.
We would also welcome practice nurses with a Non-Medical prescribing certificate & minor ailments certificate to apply.
The practice has provided practice nurse VTS training in the past.

Main duties of the job
The successful candidate would have experience and skills in the following areas:
* QuoF related Chronic Disease Management
* Smear Cytology - a must
* Removal of Sutures
* ECGs
* Venepuncture
* Ear syringing
* Routine immunisations / Childhood immunisation
* Contraception
Knowledge of national and local guidelines with the ability to apply these to individual patients and set targets is essential.
We ask for experience in dealing with the above, holding a diploma as a minimum in each chronic disease is not expected but core knowledge and ability to assess follow-up of stable patients based on QuoF requirement is a must.

Job responsibilities
Chronic Disease Management
* Experience in Chronic Disease Management.
* Asthma/COPD/Peak Flow Readings/Spirometry
* Knowledge of national and local guidelines with the ability to apply these to individual patients and set targets
* Ability to support patients with lifestyle changes
Other Health Areas
* Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
* Cytology
* Removal of Sutures
* Travel Clinic
* ECGs
* Venepuncture
* Routine immunisations / Childhood immunisations
* Chaperoning and assisting patients where appropriate who are being examined by another clinician
* Assisting GPs with minor surgery
* Requesting pathology tests, for example, blood tests, urine culture, swabs
* Contraception basic advice.
* Following agreed clinical protocols with referral to GPs as appropriate
Other tasks will include:
* Maintaining and cleaning equipment used by the nurses and GPs
* Maintaining GP and Nurses rooms, stocking and rotating items as required
* Maintain general tidiness and cleanliness of nurses and treatment rooms
* Ordering of vaccinations to maintain stock levels
* Participation in administrative systems in the Practice
* At all times there will be a need to maintain accurate records
* A duty to advise senior colleagues of potential problems or errors within a range of assigned tasks
* Attend and participate in any Practice meetings when required.
* Any other delegated duties appropriate to the post
Person Specification
Qualifications
* Registered first level nurse
* Clinical supervision
Experience
* Minimum 2 years post registration experience
* Management of long-term conditions
* Involvement in implementing and using protocols and clinical guidelines
* One year recent primary and community nursing experience Participation in quality initiatives such as clinical benchmarking
* Experience of audit
Knowledge
* Knowledge of needs of patients with long-term conditions
* Aware of accountability of own role and other roles in a nurse-led service
* Knowledge of health promotion strategies
* Awareness of clinical governance issues in primary care
* Knowledge of patient group directions and associated policy
* Ability to identify determinants on health in the local area Knowledge of public health issues in the local area
* Awareness of local and national health policy
* Awareness of issues within the wider health economy
Skills
* Clinical Skills: cervical cytology, immunisation and vaccination, ear care, minor surgery, and wound care
* Experience of teaching and mentorship in a clinical setting
* Chronic Disease Experience.
