An exciting opportunity has arisen to work in the Multidisciplinary Assessment Service (MuDAS Unit). We are a day unit based in Wycombe hospital and open from Monday-Friday. The unit provides a Comprehensive Geriatric Assessment to patients out in the community. Additionally, we have an Intravenous Infusion lounge. The aim of the unit is hospital admission avoidance in the acute settings.
As a Staff Nurse, you will need to have excellent communication and interpersonal skills with the ability to work independently but also as part of a team.
Our Multidisciplinary Team consists of doctors, nurses, consultants, physiotherapists, occupational therapists, SALT, and health care assistants, to meet the needs of the patients and avoid hospital admission. As a Staff Nurse, you will be confident and competent in drug administration, managing deteriorating patients, simple wound dressing, venepuncture/phlebotomy, utilizing advanced assessment skills, and clinical nursing skills. We care for patients with a wide variety of complex medical conditions. Our Intravenous Infusion lounge treats a number of patients for blood transfusion, iron infusion, hydration, and antibiotics. We work in close partnership with other primary health care professionals such as GPs, 111, specialist services, and medical wards in the Trust.
Key Responsibilities:
1. Assess the physical and psycho-social needs of patients and deliver a high level of therapeutic interventions based on best available evidence.
2. Manage own workload, delivering and evaluating service delivery to patients with a variety of complex medical conditions.
3. Take part in the referral management process to ensure patients are treated by the most appropriate member of the team.
4. Contribute to the development of protocols and procedures developed for clinical pathways.
5. Participate in the 5-day working rotas.
