Mold Hospital is a very busy community hospital, at the heart of the local community. We pride ourselves on the high standard of care we provide to both patients and relatives. We aim to ensure that the care patients require is accessed closer to their home, and we believe that discharge planning from the day of admission is an essential part of our role to ensure that patients are discharged at the earliest opportunity.
The staff at Mold Hospital are committed to the home first principles and have excellent working relationships with all members of the multi-disciplinary team. This enables us to implement the discharge to access principles alongside our Community Resource Team.
We offer excellent development opportunity for experienced nurses and 6 months preceptorship for newly qualified staff. The hospital has two wards and each has the capacity to care for 20 patients. We encourage staff to progress and support staff with their personal development.

Main duties of the job
The post holder is responsible for the assessment, planning, implementation and evaluation of evidenced-based nursing care, working collaboratively and co-operatively with others to meet the needs of patients and their families.
Assist in the management and organisation of their clinical area and participating in the education, development and supervision of other staff members.
The team consists of nursing staff, HCSW, domestics and administrative staff who work together to provide high levels of patient care. The team are flexible in their approach to all aspects of patient care always putting patient needs first.
The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.
