We have an exciting opportunity within the Integrated Discharge Team for a Band 7 Team Leader. The post holder will be responsible for leading discharge planning across all areas of the trust and managing a team of nurses responsible for the assessment and discharge planning of patients with complex needs.
Main Duties of the Job
1. Lead and manage the Integrated Discharge Team on a day-to-day basis, providing clinical leadership, support, and guidance.
2. Support team members in facilitating discharge plans where disputes arise.
3. Assume responsibility for the most complex cases, including leading assessments for homeless patients.
4. Assume clinical responsibility for Intermediate Care placements made by the team.
5. Ensure discharge pathways and estimated discharge dates are embedded across the trust.
6. Maintain patient records appropriately and record relevant information.
7. Collaborate with managers to implement and review the Hospital Discharge Policy.
8. Act as the designated clinical link for the Trust regarding NHS Fully Funded Continuing Care.
