We are looking for a team member to roll out a new initiative working with young people and families to prevent hospital admission. This pilot project, named the NOOK, involves developing and rolling out the service.
As a Mental Health Practitioner, you will be part of a specialist children and young people's mental health team, providing intensive Tier 4 support and interventions for young people and their families.
Main Duties of the Job
CWP (Tier 4 CAMHS) provides mental health services for children & young people with emotional, psychological, and mental health-related difficulties. Services are delivered through assessment for intensive support in a residential inpatient setting or a home-based treatment approach.
A whole systems approach is utilized in assessing and offering interventions and support. You will work as part of a team on a rota basis, including unsocial hours, and be responsible for promoting and upholding CWP Trust Values.
Your responsibilities will include undertaking the assessment of young people referred to Tier 4, coordinating assessments, recording findings, and communicating outcomes with relevant stakeholders.

Person Specification
Qualifications
* Professional Registration with a recognized National Body (HCPC, NMC, Social Work England)
* Nationally recognized post-qualification, continuous professional development (CPD) related to the core profession (Social Work, Nurse, Allied Health Professional)
* Therapeutic Training to intermediate/diploma level
Knowledge and Expertise
* Demonstrable working knowledge of specialist therapeutic approaches (e.g., CBT, Family Therapy)
* Knowledge of Child Protection workings and Practice
* Working knowledge of Children related legislation
* Experience of inter-agency working
* Experience of writing, implementing, and evaluating Care/Treatment Plans
* Experience of working with Students/Learners
* Experience of providing clinical supervision
Experience
* Significant post-qualification experience of working within a CYPMHS or Child Mental Health related field
* Recognized experience of working in an environment where evidence-based interventions have been delivered
* Undertaking formal and informal clinical risk assessments and risk management planning
* Undertaking mental health assessments
* IT skills, including using databases, Microsoft Office, and clinical systems
* Post-qualifying experience of working within NHS community CYPMHS
