Position: Social Worker - Community Integrated Response Team (CIRT)
We are currently advertising for a Social Worker within the Community Integrated Response Team (CIRT); Urgent Community Response (UCR) - a 2-hour response service. The UCR team is co-located with NHS colleagues at St Catherine's Health Centre and works collaboratively across a multi-disciplinary team, including Nurses, Occupational Therapists, Physiotherapists, Community Paramedics, Virtual Frailty Ward professionals, Health Care Assistants, and Social Care. This admission prevention service provides urgent assessments, care, and treatment at home for individuals at imminent risk of hospital admission.
Role Responsibilities
The successful candidate will support the delivery of the Personalisation and Inclusion Pathways for Adults and Carers, ensuring compliance with the legal framework of the Care Act 2014. Wirral Council employs the 3 Conversations model (3C’s) across all Adult Social Care teams, focusing on identifying individuals' strengths and aspirations to maximize independence and wellbeing. Working Hours
36 hours per week, with various shift patterns (7.2-hour days) covering 8am-8pm, 7 days a week, including weekends from 9am-5pm on a rota basis.

The Council has a visa sponsorship license and can offer certificates of sponsorship to non-UK residents through the skilled-worker route. Applicants must be registered with Social Work England and require sponsorship. Note that sponsorship alone does not grant a visa; applicants must still apply and receive their visa to have the Right to Work in the UK.
