To work as part of the Intermediate Care Services (ICS) Discharge Team contributing towardseffective, safe, timely discharges of patients from the acute setting To manage a complex case load of patients with responsibility for theirdischarge planning from admission to ensure discharges are expedited in atimely and safe manner. To provide leadership in the promotion of rehabilitation, patient independenceand therefore earlier discharge from hospital Effectively monitor patient progress towards EDD, taking action to ensuredelays are promptly addressed by the relevant ward or department. Contribute to the effective running of the ward by concentrating on tasks,which directly support and assist the safe and timely discharge of the patients.This will include completing discharge planning documentation,communicating with external agencies such as social services, patientsfamilies and carers, arranging Work closely with members of all areas across the hospital including being the point of contact
for the Emergency Department and the Acute Medical Unit to lead facilitating discharges of non FES patients from these areas that have care needs or from nursing residential homes to avoid admissions To contribute to the local resolution, investigation of any informal or formal complaints and carry out follow up action for those that specifically relate to discharge planning
