Job Introduction
As a Acute Care at Home Practitioner, you will play a key role in delivering high-quality, person-centered care and services to support individuals and teams. Working within a collaborative environment, you will provide essential services tailored to meet the unique needs of those we serve.
Based in Swindon, you will have the flexibility to work within a model that supports both on-site and hybrid working arrangements, depending on the nature of the role. This role requires flexibility in response to changing healthcare needs. Responsibilities may be adjusted in consultation with the post holder to reflect service demands and improvements.
The role follows a flexible rota to ensure comprehensive service coverage, including weekends, evenings, and Bank Holidays. Work schedules will be reviewed periodically to align with patient needs.
Main Responsibilities
1. Delivering urgent and planned community-based care for acutely unwell patients.
2. Conducting physical and clinical reasoning assessments (PACR or equivalent) to inform diagnosis and treatment plans.
3. Developing and implementing individualized care plans in collaboration with patients and their families.
4. Supporting leadership, management, and the organisation of the Hospital at Home team.
5. Communicating effectively with patients, families, carers, and multidisciplinary teams to ensure continuity of care.
6. Providing emotional and psychological support for patients and carers in acute, chronic, and end-of-life situations.
7. Contributing to service improvements and ensuring efficient resource management.
The Ideal Candidate
1. Registered Nurse (NMC Registered) or HCPC Paramedic.
2. Physical assessment and clinical reasoning qualification (PACR) or equivalent.
3. Broad clinical experience transferable to the community setting.
4. Strong organizational, communication, and interpersonal skills.
5. Experience in leading, supporting, and developing teams.
6. Ability to communicate effectively with diverse patient populations.
7. Skills in teaching and assessing in clinical practice.
8. Systematic approach to assessment, planning, and evaluation of care.
