Herefordshire and Worcestershire Health and Care NHS Trust
Part time 15 hours 18:30-23:30 over 3 evenings.
This role is a Band 5 Community Staff Nurse, as part of the evening team working 18:30-23:30 over 3 evenings a week.
The team is a countywide team which is centrally based in Droitwich with satellite bases in Evesham, Bromsgrove and Kidderminster. This post is primarily based in Droitwich covering South Worcestershire. Nurses visit patients in their own home, supporting them in remaining at home, preventing admission to hospital, and supporting early discharge from hospital.
The post is all worked as unsocial hours, hence picking up pay enhancements for all hours worked, making this post more attractive. More money for less hours!
These working hours offer perfect work-life balance, particularly if you have a young family. You will have the daytime to enjoy hobbies and activities but remain in a progressive career.
We pride ourselves on providing outstanding care to patients across Worcestershire. We will provide you with resources to develop and progress a long-term career, as well as flexibility, continual professional development, and a wide range of wellbeing and support. If you would like to discuss informally if this is the right role for you, get in touch with the Recruiting Manager, whose contact details can be found on the top right-hand side of the advert.
Main duties of the job
As a Community Nurse working with the Evening Team, most of the work allocated to you will be of an unplanned nature. The service works as an on-call service covering the County of Worcestershire, taking direct referrals from patients, 111, Ambulance Service, and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to then work alone.
The evening service nurses primarily work alone and have work allocated via a handheld tablet by the triage nurse based at Droitwich. You will be able to start your visits from home if the geography dictates.
The patients seen during the evening and night vary immensely; no two shifts are the same. For example, we see patients with catheters, palliative care needs, wound care, administration of medication, diabetes management, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention. As a team, we often undertake initial assessments of patients in the community.
As part of a new initiative, we are working closely with the West Midlands Ambulance Service and 111 to assist in responding to urgent care responses. These can be patients that have fallen or patients requiring further assessment to aid prevention in admission to hospital.
