We are looking for someone who can work Monday to Friday, with some early mornings and weekends. Your role will involve to provide short-term, intensive, hands-on support, direction and information to eligible families who need intensive family support To provide effective support to the Lead Professional who is coordinating services, as part of a whole family approach and contribute to/engage in the development and effective delivery of the family action plan as appropriate. To help families define their own goals, responding to individual needs and support the family to navigate and access relevant services. To offer positive role modelling, practical and emotional support to families to improve independence and well-being. This You will be required to travel within your designated area of work, a full driver’s licence and your own transport are essential.
