Job Summary
We are seeking a Mental Health Nurse, Social Worker, or Occupational Therapy team leader to join our developing primary care Mental Health Team in the East of Southampton. You will be based within GP surgeries and work with service users of adult working age, including older persons with mild to moderate mental health difficulties. You will work within your scope of practice, so if you have worked within specialties, these skills will be transferable.
The successful applicant will become part of a growing primary care team that delivers care to some of the most diverse populations in Southampton. The PCN is a large, inclusive team with a focus on multi-disciplinary working. You will need to have a flexible, multi-disciplinary approach, a good understanding of risk assessment, crisis planning, and the recovery model.
There will be training available for the post, and part of the role will work towards delivering group interventions using the Comprehend, Cope, and Connect model (CCC) within the PCNs - an opportunity for a new approach to mental health.
Main Duties, Tasks & Skills Required
The team leader role is largely clinical; you will contribute to the triage service, offering early assessment, signposting, brief interventions, and group delivery for people with mental health needs in their local communities. You will play an active part in the development of systems and processes that support care delivery within these teams. You will require good communication skills and be able to build positive partnerships with key stakeholders in primary and secondary care. You will work collaboratively with Solent Mind and have peer recovery workers working alongside practitioners in the surgery.
The role requires excellent communication skills, a high standard of risk assessment skills, partnership working, and the ability to work in a clinic environment within the GP surgeries.
