An exciting opportunity has arisen to join the Practice Development Nurse in Area Central Team based in Bron Afon, Llanfairfechan on the North Wales Coast.
The successful candidate will take a lead role on specific work streams relating to Mental Health and Dementia care, as well as general nursing care in care homes. They will work collaboratively across Conwy and Denbighshire to implement programs aligned with national and health board objectives.
The main focus of this role is improving patient care and safety through various approaches, supporting staff in care home nursing settings, and possibly involving other multidisciplinary areas as defined by the Health Board.
The candidate will provide professional and clinical leadership within EMI and general care homes, offering clinical advice, care planning, and education on Dementia, mental health, and physical health issues to healthcare professionals across both counties.
Main duties include:
* Developing and maintaining communication networks with stakeholders to foster professional behaviors and skills.
* Facilitating education and training for nurses and healthcare assistants, aligned with standards and area needs.
* Promoting patient safety and reducing clinical risk through educational programs and skills assessments.
* Supporting delivery of high-quality, safe, and compassionate healthcare, demonstrating BCUHB's values.
Welsh language skills are desirable; applicants who speak Welsh and/or English are encouraged to apply.
