An exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to join our highly respected and friendly practice team. Working 16 - 24hrs per week.
We are looking for a dynamic and committed Advanced Nurse Practitioner to work as part of the clinical team to support the delivery of high quality healthcare services to our patients. Experience of independent working in primary care and having a prescribing qualification is essential.

Main duties of the job
The Advanced Nurse Practitioner will be expected to undertake all duties and responsibilities associated with an Advanced Nurse Practitioner working within Primary Care, including (but not limited to):
* Working as an autonomous, advanced practitioner, with an extended scope of practice.
* Supporting our experienced team to deliver care to patients in face to face consultations, telephone appointments and within their own homes on occasions.
* Providing initial history taking, clinical assessment, diagnosis, treatment plans for patients using evidence-based principles and current best practice.
* The role will involve independent prescribing within the scope of professional practice.
* Keeping comprehensive, accurate records that support the clinical reasoning process in the assessment and treatment of all patients.
* Working as part of a multi-disciplinary team, sharing clinical knowledge and experience.
* Ensuring all clinical practice is based on current clinical evidence.
* Demonstrating through day-to-day action a contemporary approach to Primary Care which is patient-centred, prudent and team-enabling.

Job responsibilities
Clinical responsibilities
* Diagnosing and treating patients presenting with minor illness.
* Ensuring clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
* Provide first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of advanced skills in history taking, physical examination, problem solving and clinical decision making to establish a diagnosis and management plan.
* Instigate necessary invasive and non-invasive diagnostic tests and investigations and interpret findings and action within own competence.
* Prescribe and review medication for therapeutic effectiveness, in a safe, effective and appropriate manner as defined by current legislative frameworks and local guidelines, and within scope of practice.
* Provide safe, evidence-based individualised care to the practice population.
* Refer patients directly to other services/agencies as appropriate.
* Work with the multi-disciplinary team within the practice and contribute to practice targets (local and national) with disease management agenda. E.g. QoF.
* Undertake scheduled home visits for patients unable to attend the surgery as agreed with the Duty GP and within scope of practice.
* Support other healthcare professionals within the practice and offer advice and guidance within area of competence.
Administration and professional responsibilities
* Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.
* Ensure the clinical computer system is kept up to date with accurate details recorded and amended.
* Dealing appropriately with results and clinical letters for patients under your care.
* Ensure accurate completion of all necessary documentation associated with patient health care and registration.
* Attend and participate in practice meetings as required.
