Due to the expansion of our Overnight Community Nursing Services, an exciting opportunity has arisen for an experienced community nurse to work as part of a wider team covering Urgent Community Nursing Care for West Sussex. We are offering an opportunity to work overnight from 23:00hrs to 08:00hrs from a base in Chichester alongside a Healthcare assistant. You will be part of a team with bases in Arundel/Lancing, Brighton, and Crawley. All teams cross cover the areas to ensure the quickest response to patients in need of urgent care.
Main duties of the job
Providing urgent care to patients requiring care in the community, including care of End of Life patients, those with catheter-related problems, and any other urgent care needs.
A pool car will be available, and you will be expected to drive it; however, on occasions, you may need to use your own car. You will need to be confident driving at night.

Job responsibilities
We are seeking a highly experienced, confident professional with some experience within the community and the necessary skills in managing the complex clinical needs of our patients, i.e., End of Life support and symptom management, chronic disease management, and avoiding admission to the Acute hospitals within our area.
Person Specification
* Holds a valid UK driving licence and is comfortable driving at night.
* Minimum 3 years post-registration experience.
* Evidence of independent working and clinical decision-making.
* Community nursing experience in the UK.
* Confidence liaising with members of other professions to develop a plan for safe community care.
* Wide experience of managing complex nursing care to include end of life care, catheterisation, urgent wound care, complex assessments.
* Community Specialist Practitioner.
Leadership
* Experience of leadership.
* Qualification in leadership.
