Community Clinical Nurse Reporting to: Registered manager Job Description Hours per week: 08.00-17.30 Location: Community based (you might be required to work night shifts such as shadowing shifts, sign off’s, spot checks), WFH, 1 day a week in Liverpool St office preferably on Tuesday’s WE ARE LOOKING FOR A NURSE IN THE LONDON AREA AND ONE IN HERTFORDSHIRE Aim of the role Conducting initial assessments of service users, develop individualized care plans in collaboration with patients, families, and healthcare professionals. Ongoing care plan reviews and risk assessments Observing care staff and ensuring their competency Managing clinical paperwork, including Medication Administration Records (MARs) – monthly audit Providing general clinical oversight of the branch and service users Occasionally providing direct care to service users to maintain hands-on clinical skills (e.g. tracheostomy change / sign off carer competency at same time) Main duties: Shadowing & sign off competencies for candidates Initial assessments & implement care plan for individual service users Care plan & RA reviews – shared task between Community and Clinical lead nurse Medication audit – monthly, via Birdie Quarterly spot checks Building rapport / regular check in with parents – Establishing effective working relationships with parents, their families and carers. This will include promoting individual rights and recognising and respecting their contributions to care planning and delivery. Working with multi – disciplinary team Ensuring admin is up to date Working closely with the Clinical Lead nurse to ensure care delivery is of a high quality and meets required standards Support and Deputise for the Clinical lead Nurse and provide knowledge and skills to support care delivery.
