Job summary An exciting opportunity has become available to join the Valentine Health Partnership. We are looking to recruit an enthusiastic, highly motivated and experienced Clinical Pharmacist to join our General Practice Team who acts within their professional boundaries, supported and working alongside a team of general practice pharmacists and technicians. The post holder will work as part of a multi-disciplinary team and provide clinical pharmacy support to the practice/PCN with regards to prescription and medication queries. They will ensure the practice integrates with community and hospital pharmacy to help utilise skill mix to improve patient outcomes and ensure better access to healthcare. They will play a key role in enhancing medicines use and the management of Long Term Conditions (LTC). The Post holder will provide the patient-facing services within the practice including undertaking medication reviews for complex patients with polypharmacy issues. The post holder will work with SEL ICB in supporting and development of policies. The role is pivotal to improving the quality of care and operational efficiencies and so requires motivation and passion to deliver excellent service within general practice.
