As a Healthcare Assistant at St Lukes Hospice, you will contribute to delivering exceptional care and support to patients and their families. Your responsibilities will include: Clinical Care Provide personal care to patients, including assistance with hygiene, mobility, and nutrition, ensuring their comfort and dignity at all times. Monitor and report changes in patients conditions to the nursing team promptly. Safely handle patients in line with current moving and handling regulations. Assist with the maintenance of a safe clinical environment, ensuring adherence to health, safety, and infection control standards. Participate in the effective use of resources, including equipment and supplies. Support for Patients and Families Build trusting relationships with patients and their families, offering reassurance and understanding during difficult times. Provide emotional and practical support to carers and visitors. Assist with end-of-life care, including support after a patients passing. Team Collaboration Work under the supervision of registered nurses as part of a multidisciplinary team to deliver holistic, patient-centered care. Actively support and guide new staff members during their induction and training. Contribute to team meetings and discussions, sharing insights and observations to improve care delivery. Professional Development Identify your own learning needs and participate in ongoing training, education, and development opportunities. Stay informed of hospice policies and procedures, ensuring your practice aligns with organizational standards. Administrative and Communication Duties Communicate effectively with patients, carers, and all members of the multidisciplinary team, both verbally and in writing. Input and update patient information in the electronic database system as directed by the registered nurse. Follow hospice policies on confidentiality and safeguarding, ensuring sensitive information is protected.
