Job summary An opportunity has arisen at Greens Norton & Weedon Medical Practice for an enthusiastic, committed & caring Advanced Nurse Practitioner (ANP) to join our well-established two site practice for 30 hours per week (over 3-4 days). Working within the NMC Code of Conduct and utilising primary care skills and experience, working as part of our duty team, the post holder will provide advanced nurse interventions and influence the development of nursing services. Main duties of the job Working autonomously, the ANP will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. Key responsibilities: Assess and triage patients, including same day triage (working as part of our duty team) and as appropriate provide definitive treatment, including prescribing medications following policy, patient group directives, NICE guidelines and local care pathways, or make necessary referrals to other members of the primary care team. Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person. A flexible approach is a must as we deliver face to face and telephone consultations as well as visiting housebound patients.
