Are you an experienced Tobacco Dependence Adviser or Healthy Weight Practitioner? Live Well works with Croydon residents to support their behaviour change journey and make a difference to their overall health and wellbeing. We are passionate about what we do. We are a small team providing 12-week evidence based, client centred, healthy lifestyle interventions to help our residents quit smoking, lose weight, or reduce their alcohol consumption. We offer face to face, telephone or video call support and occasional small group work. We are looking for skilled and experienced practitioners passionate about delivering high quality lifestyle interventions with clients in one or more of the above areas. We can provide training in other areas. You will also contribute to our MECC - Making Every Contact Count roll out. You will use your skills in motivational interviewing, behaviour change and coaching to support clients to set personalised goals; helping them to overcome barriers to behaviour change and to succeed. You will manage your own caseload and be competent in using client management databases to monitor and record activity and progress.
