About the role
We are looking for enthusiastic and motivated individuals to join the existing Social Prescribing Link Worker team to work in partnership with primary care, mental health and specialist drug, alcohol and gambling services. Social Prescribing Link Workers empower people to take control of their health and wellbeing, focusing on ‘what matters to me’. Working closely with the wider clinical teams, this role takes a holistic approach to an individual's health and wellbeing, assisting clients to overcome a wide range of social, emotional and practical challenges. The successful candidate should have experience of working positively with people; inspiring trust, building confidence and motivating. You will have great interpersonal skills; be empathetic, resilient and be able to maintain effective working relationships with partners across the sectors.
