The Frailty Virtual Ward is a Geriatrician led, multidisciplinary team, with integrated pathways and collaborative working across acute and community services.
This role offers the opportunity to help shape the future development of this new and emerging service in York, providing acute care to patients within their own home environment, supporting early hospital discharge and admission avoidance. The department and Trust both offer a welcoming and dynamic working environment.
The post holder will be based in York and will require the ability to travel across the footprint of Community Services. It is essential that the post holder holds a full UK driving licence and has access to their own vehicle. Mileage will be reimbursed.
There are no on-call requirements for this position and core working hours will essentially be daytime working. The current service operates Monday to Friday 8am till 4pm; however, the future ambition is to provide a 7-day service. This model may therefore move to include weekend working with appropriate remuneration.
This post can be either full or part-time and will be offered for a fixed term of 12 months.
We are looking for the post holder to start in August; however, there is flexibility regarding the exact date.
Main duties of the job
The post holder will be expected to assess patients in their own home environment while also working collaboratively within the MDT to devise holistic management plans.
Day-to-day duties include triaging referrals, carrying out comprehensive geriatric assessments, performing diagnostics, creating treatment plans, and referring to specialty colleagues as appropriate.
We are looking for a friendly, enthusiastic, and innovative individual who will work well with the team to meet service demands. We are seeking individuals with a keen interest in frailty and wanting to develop their skills and expertise with a view to formal training in geriatrics or as an extended role in general practice.
As this is a new and expanding service, there will be numerous opportunities for the post holder to get involved in quality improvement projects to further develop the Frailty Virtual Ward.

DBS Checks
Please be aware that all new employees starting work with us will be charged for the cost of their DBS check if it is required for the role. The cost will be deducted from your salary either in 1 payment or over a six-month period once you start employment. By applying you are agreeing to this undertaking.
