ABL Health will soon be launching a new Healthy Lifestyle service in Gloucestershire, commissioned by Gloucestershire County Council. This transformative service seeks to work collaboratively with communities to transform the lives of the local population. The service will support residents to help them lose weight, stop smoking, reduce alcohol consumption, get more active and improve their wellbeing - all embedded in sustainable behaviour change. The service will also deliver the 1001 days programme supporting families and young children and the Healthy Workplace award scheme. You will work with a particular focus on our 1001 day programme, supporting people during pregnancy and early parenthood with weight management, smoking cessation, reducing alcohol consumption (accredited training can be provided) as well as working with this community to co-produce the development of the programme to meet their needs. The ideal candidate will be passionate about promoting healthy lifestyles and delivering client-centred motivational change. You will be passionate about improving maternal and infant health and wellbeing, have a background in nutrition, exercise, health improvement, adult and/or family weight management, smoking cessation or similar and be an excellent team player and have a willingness to learn and develop. Duties and Responsibilities Deliver ABL Healths Healthy Lifestyles service, through one-to-ones, in the community, and through digital methods such as Zoom/Teams or phone appointments. Deliver evidence-based education around nutrition, physical activity and behavioural change as advised by ABL Health team and in line with ABL Health programme curriculums. Deliver programmes with a client-centred approach using skills of positive psychology and motivational interviewing. Deliver the 1001 days service Monitor client's progress through regular consultation and with the use of Microsoft excel, identifying specific treatments for those not on track to achieve their goals. Work with clients and stakeholders to co-produce in-session materials Contribute to the contract performance report by submitting regular case studies to the project lead. Contribute to and support ABL Healths social media pages. Contribute to training and support new staff and volunteers. Working with individuals and groups using SMART goal-setting. Keep detailed files, recording data accurately, ensuring all forms and other documentation is completed correctly. Have a thorough understanding of ABL Health policies and procedures. Engage and recruit potential clients at promotional and community events. Contribute to ongoing service development needs. Build community links and partnerships within LMNS working to encourage new referrals into the service and maintain close working relationships with other community services. Present service and referral pathway information at stakeholder meetings as directed by the Head of Service To attend and actively participate in team and wider company meetings, such as MDT. Be able to organise own workload in order of priority and deadline dates, daily administration tasks and other work-related activities. To demonstrate personal and professional development in line with the ethos of lifelong learning To work within the framework of clinical governance to maintain and improve patient care, quality and efficiency. The postholder will be required, when and where appropriate to the role, to comply with the processing of requests under the Freedom of Information Act 2000 The postholder must comply with ABLs policies that protect the information assets of the organisation from unauthorised disclosure, modification, destruction, inappropriate access or use. The postholder will be responsible for maintaining the clinical and/or corporate records that fall within the remit of this role to the standards in ABLs records management policies, and data quality processes and standards. Health & Safety Compliance with the Health & Safety at Work Act 1974 the postholder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Training The postholder must attend any training that is identified as mandatory to their role.
