To provide Health Care Assistant services to the practicepopulation
Supported by the nursing team within the practice, theHealth Care Assistant (HCA) will deliver care within the boundaries of their role and inaccordance with specific practice guidelines and protocols.
Main duties of the job
Responsible for the care delivered, demonstrating critical thinking and skills in clinicaldecision making within the parameters of their qualifications and role requirements.
They will focus upon supporting patients to be healthy by assessing their needs, providingHCA care, health education advice, screening activities, implementing care at the surgery, community or other settings, maintaining clinically accurate records and running clinics.
They will work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy and procedures, and providing HCA guidance as required.

Our practice is a a training practice running from 2 locations. The role will be based at our practice in Finch Road Primary Care Centre.

o Supportpatients to adopt health promotion strategies and encourage principles ofself-care
o Offerholistic service to patients and their families, developing where appropriate an on-going plan of care/support,with an emphasis on prevention and self-care
o WeightManagement & Advice
Venepuncture
Chronic Disease management
o Undertakeappropriate investigation, clinical observations and interventions following agreed guidelines forpatients with chronic diseases CHD, Diabetes and Hypertension, Asthma, CKD
Emergency Care
o Prioritise health problems andintervene appropriately to assist the patient incomplex, urgent or emergencysituations, including initiation of effective emergency care.
Wound Management
O Removalof stiches and clips
o Meet theneeds of patients presenting for wound care and use dressing formulary
Mental health, adult and child safeguarding
o Recognisepatients presenting with mental health needs in accordance with the National Framework (NSF) forMental Health. Raise relevant information with the duty GP.
o Recogniseadult and child safeguarding situations and act appropriately
Learning Disability annual check withclinician/GP
Vaccination programmes
o Providevaccination and immunisation programmes for Adults following guidelines for flu, pneumovax, shingles, B12
Infection Control
o Monitorfridge temperatures twice daily
o Managevaccination stock when required
o Checks ofemergency trolley, home visit bag and any other check as required
Knowledge of IT
O EMIS web
o Docman
o ICE
o Anyother package required with in the Practice
Patient Profile Activities
O Reviewpatient data to work with the nursing team, to achieve focused patient healthcare
o Activeparticipation in achieving QOF and Enhanced Services targets and recall demands. Contribution to theimplementation of Services
Person Specification
Experience
Essential
1. Must already work as a Health Care Assistant in General Practice
2. NVQ in Health and Social Care qualification L3
3. GCSE in English and Maths
4. Effectively manage own time, workload and resources
5. Reflect on own and team activities to identify risks, enhance performance and improve quality of patient care
6. Work within own competence
7. Produce accurate, specific and complete records of patient consultation
8. Deliver care according to the NSF and the NICE guidelines and evidence based care.
9. Understand and apply statutory procedure and local guidance regarding the identification of vulnerable adults and children, domestic violence, substance abuse and addictive behaviour
