To undertake a variety of duties including:
1. Surgery consultations
2. Telephone consultations and queries
3. Visiting patients at home
4. Checking and signing/authorising repeat prescriptions
5. Dealing with queries, paperwork, and correspondence in a timely fashion
Manage a broad range of acute and chronic illnesses, ensuring that care is delivered in line with evidence-based guidelines and best practices. Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assess the health care needs of patients with undifferentiated and undiagnosed problems.
Screen patients for disease risk factors and early signs of illness. Participate in the management of chronic diseases, such as diabetes, hypertension, asthma, and COPD, ensuring that patients receive optimal long-term care. In consultation with patients and in line with current practice disease management protocols, develop care plans for health. Provide counselling and health education.
Record clear and contemporaneous consultation notes to agreed standards. Collect data for audit purposes. Compile and issue computer-generated acute and repeat prescriptions. Prescribe in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
The post-holder will be required to visit patients during surgery time in an emergency if clinically indicated. The post-holder will undertake a share of home visits per week as allocated.
