An exciting opportunity has arisen for a full-time Nursing Associate to join the Marlow Adult Community Healthcare Team.
The successful candidate will be part of the friendly and proactive community team, working collaboratively to provide a high standard of nursing care in the community.
We offer a preceptorship programme for newly qualified nursing associates, an extensive orientation programme, and support from advanced community practitioners and the practice development team.
Training and support will be provided to achieve your competencies.
Responsibilities:
1. Support the Community Nurses in a wide range of nursing activities, including admission prevention, facilitating discharges from hospital, and end-of-life care.
2. Conduct holistic patient assessments, administer medication, provide wound and pressure area care, and manage catheters.
3. Provide compassionate, safe, and effective care and support to people in various care settings.
4. Contribute to ongoing assessment of patients, recognizing when to refer and escalate to a Registered Nurse for reassessment.
5. Monitor the condition and health needs of people in their care continually, in partnership with patients, families, and carers.
6. Record all care delivered appropriately and contemporaneously, ensuring documentation is clear, concise, and legible, and signed and dated.
7. Communicate effectively with colleagues, providing clear verbal, digital, or written information and instructions.
8. Communicate effectively with patients when delivering care, providing advice when appropriate.
9. Recognize and report any situations, behaviors, or errors that could result in poor care outcomes.
10. Understand, encourage, and complete the necessary paperwork to enable safe and timely discharges/transfers.
