Chertsey HealthCentre is seeking a Practice Nurse to join our wonderful team.
As a Practice Nurse, you will work alongside our multi disciplinary team to ensure the delivery of the highest possible levels of care to all patients.

Our list size is around 22,000 patients and we have a large team providing patient care including 6 GP Partners, 8 salaried GPs, GP registrars, ANP, Clinical Pharmacists, Practice nurses, HCA's, GPA's, paramedics and a phlebotomist. We also have a great team of administration/reception staff to support the clinical team.

Job responsibilities
To be responsible for the implementation ofprocesses for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.
Working as a senior member of the practicemultidisciplinary team, the post holder will ensure nursing services aredelivered effectively to the entitled patient population.
The Practice Nurse will be responsible for anumber of clinical areas such as infection prevention andcontrol, health promotion, chronic diseasemanagement, as well asactively supporting the practice management team in the reviewing and deliveryof clinical policy and procedure.
